Supported Living is a county board funded service provided to individuals with short term or onetime cost funding to support individuals in their current homes in Sandusky County, provide emergency stabilization assistance, prevent the need for individuals to have more restrictive settings, or to maintain their health and safety. The funds can help offset extraordinary housing costs, equipment needs, direct support staff in home settings, respite services beyond what can be funded through Family Support Services, among other kinds of supports. All services must be prior approved by the superintendent as urgently necessary, must be of a short-term nature, and must prevent the need for more significant intervention by use of the funds. The amount of funds are determined by the Board annually and are subject to change based on availability of local funding. To receive supports, individuals must currently be determined eligible for county board of developmental disabilities services.
Supported Living providers must be eligible for payment through the Sandusky County Auditor’s Office, be certified through the Ohio Department of DD for the applicable services, and are subject to review by the Ethics Council of the Sandusky County Board of DD. To request assistance, individuals or their natural support people may contact their Service and Support Administrator for assistance.
